This website is intended for business-to-business inquiries from dental practices and related organizations. Business contact information voluntarily submitted may be used to respond to the inquiry, discuss DentalVerify Pro services and manage prospective or client relationships.
Do not submit patient information publicly
Do not submit patient names, dates of birth, insurance member IDs, treatment details or other patient information through this public website, ordinary contact form or initial email. Patient information should only be exchanged after onboarding through the approved workflow established with the dental practice and subject to applicable agreements.
Third-party form processing
The public inquiry form is processed using Formspree. Only business contact information should be submitted through that form. Review Formspree's own terms and privacy information before using the form if you have questions about its processing.
